Job Description

The Wound Care Nurse Practitioner (NP), supervised by the Director of Nursing, provides specialized wound care services to patients in home settings, nursing homes, and assisted living facilities.

  • They perform comprehensive assessments, develop individualized care plans, and manage treatment for complex wounds, pressure injuries, and surgical wounds.
  • Responsibilities include patient and caregiver education, supervising nursing staff, and collaborating with healthcare teams to promote healing outcomes.
  • The role requires a Master’s or Doctoral degree in Nursing, current licensure, Wound Care Certification within 6 months, and at least 2 years of relevant experience.
  • Physical demands involve mobility, lifting, and exposure to various environments, with travel within the region necessary.
  • Strong assessment, communication, and interpersonal skills are essential, along with adaptability and professionalism.
