Job Description

We are seeking a compassionate Nurse Practitioner or Physician Assistant to provide primary care in senior living and skilled nursing facilities.

  • Perform assessments, develop treatment plans, and manage chronic conditions.
  • Conduct routine rounds, monitor residents, and address health concerns.
  • Coordinate care with staff, families, and healthcare providers.
  • Educate residents and families on wellness and disease management.
  • Maintain accurate documentation and ensure compliance.
  • Participate in quality improvement initiatives.
