Job Description

Seeking a compassionate Nurse Practitioner or Physician Assistant for a hybrid role focused on preventive care, AWVs, HCC recapture, and medication reconciliation.

Responsibilities include conducting AWVs, documenting chronic conditions, patient education, and collaborating with care teams to improve outcomes.

Qualifications:

  • CA RN, NP, or PA licenses with furnishing and DEA licenses
  • Valid BLS and ACLS
  • Minimum 2 years clinical experience, preferably in primary or HCC-focused care
  • Knowledge of Medicare Advantage and risk adjustment
  • Proficient in EMR documentation and coding
  • Strong communication skills
  • Preferred: CRC certification, bilingual, mobile care experience
