Now hiring a full-time or part-time Nurse Practitioner or Physician Assistant in Grand Rapids, MI, to join a patient-centered practice blending Internal and Functional Medicine.

_About the Role:_
Perform comprehensive evaluations in outpatient and Skilled Nursing Facility settings, manage acute and chronic conditions with a holistic approach, order and interpret diagnostic tests, develop personalized treatment plans, prescribe medications and lifestyle interventions, collaborate with care teams, and educate patients.

_Qualifications:_
Licensed NP or PA in MI, board-certified in Internal or Family Medicine, preferred experience in SNFs, with interest in Functional Medicine. Strong communication and organizational skills essential.
