Now hiring a full- or part-time Nurse Practitioner or Physician Assistant for a patient-centered practice in Grand Rapids, MI.
Provide comprehensive evaluations and manage acute and chronic conditions in outpatient and Skilled Nursing Facility settings. Order and interpret diagnostic tests, develop individualized treatment plans combining conventional and functional medicine, and prescribe medications and lifestyle interventions. Collaborate with care teams and educate patients to promote self-management. Qualifications include licensure, board certification, and preferred experience in post-acute care; interest in functional medicine is a plus.
