Job Description

We are seeking a full-time APP for our mobile geriatric practice focused on wellness and disease management in assisted living communities. Our team emphasizes teamwork, support, and autonomy in a caring environment.

Our primary care services are delivered directly at assisted living facilities in Danville, Arcola, and Champaign, visiting one community daily and covering multiple facilities weekly. Each community is supported by a full-time LPN, with APPs typically overseeing 2-4 communities.

This innovative practice has grown over 10 years across Indiana and Ohio, expanding into Illinois, Kentucky, and West Virginia. We are a preferred provider and industry leader, with a strong reputation and ongoing growth.
