Nurse Practitioner/Physician Assistant – Palliative Care

Job Description

Seeking a compassionate Nurse Practitioner or Physician Assistant for our Palliative Care team.

The NP/PA will work independently and with the interdisciplinary team to provide patient-centered care focused on symptom management and quality of life.

Responsibilities include assessments, care plan development, regulatory visits, and facilitating hospice transitions.

Qualifications:
– Master’s in nursing (NP) or a relevant degree (PA) from an accredited program
– Active California licensure (NP, RN, or PA)
– Minimum two years of advanced practice experience
– Strong assessment, communication, and organizational skills
– Proficiency with electronic records and reliable transportation

Preferred:
– Prior palliative care experience
– Board certification (NP or PA)

This part-time position offers competitive pay ($59.52–$72.75/hr), benefits including health insurance, 401(k), paid time off, and flexible schedule.
