Seeking a compassionate Nurse Practitioner or Physician Assistant to join the Palliative Care team.

The role involves independent and collaborative patient-centered care focused on symptom management and quality of life. Responsibilities include assessments, care plan development, regulatory visits, and facilitating hospice transitions.

Requirements include active California NP, RN, or PA licensure, a minimum of two years’ advanced practice experience, strong assessment and communication skills, and proficiency with electronic medical records. Preferred qualifications include prior palliative care experience and relevant board certifications.
