Nurse Practitioner/Physician Assistant – Palliative Care

Job Description

Seeking a compassionate Nurse Practitioner or Physician Assistant for our Palliative Care team.

The NP/PA works independently and with the interdisciplinary team to provide patient-centered care focused on symptom management and quality of life.

Responsibilities include assessments, care plan development, regulatory visits, and facilitating hospice transitions.

Requirements:
– Master’s in Nursing (NP) or Bachelor’s degree and PA program completion
– Active California NP/RN or PA license
– Minimum two years’ advanced practice experience
– Strong clinical, communication, and organizational skills
– Proficiency with electronic records
– Valid driver’s license and reliable transportation

Preferred:
– Palliative care experience
– Board certification (NP or PA)

Position offers $59.52-$72.75/hr for 20-29 hrs/week, with benefits including health insurance, retirement plans, paid time off, and more.
