Perform patient rounding and management at SNFs, ALFs, and LTACHs.
Conduct assessments, develop care plans, monitor treatment, and adjust interventions.
Collaborate with physicians, staff, and families to ensure high-quality, coordinated care.
Complete documentation via personal laptop, with flexible scheduling and no weekends.
Receive intensive specialty training to become a subject matter expert.
