Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide equitable, value-based primary care for Medicare adults in outpatient settings.

Perform routine and acute office visits, wellness exams, and health assessments.

Coordinate care with specialists and support care team efforts, including outreach and education.

Build trusting, long-term patient relationships to promote health and prevent hospitalizations.

Participate in population health initiatives and community outreach.

Work independently while collaborating with a multidisciplinary team, including physicians and support staff.

Qualifications include: a master’s or DNP degree, national certification in family, adult, or gerontology primary care, active state license, and DEA registration. Preferred candidates have primary care experience, understanding of HCC and ICD-10 coding, and bilingual skills.
