Job Description

We are seeking a Nurse Practitioner to deliver high-quality, equitable primary care to adult Medicare patients in an outpatient setting.

  • Provide independent patient care, including routine and acute visits, wellness exams, and health assessments.
  • Collaborate with a multidisciplinary team and coordinate care with specialists and support staff.
  • Lead population health initiatives and educate patients on health conditions and treatment plans.
  • Participate in outreach, phone triage, and home visits as needed.

    • Qualifications include a Master’s or Doctorate in Nursing, national certification in a relevant specialty, active state license, and DEA license. Preferred experience in primary care and familiarity with HCC and ICD-10 coding are advantageous.
