Job Description

The Nurse Practitioner role involves providing equitable, value-based primary care to Medicare adults in outpatient settings.
They independently see patients, conduct routine and acute visits, and lead care coordination and population health initiatives.
The position includes performing wellness visits, educating patients, participating in outreach, and conducting home visits as needed.
The role reports to the Center Medical Director and collaborates with a multidisciplinary team.
Qualifications include a master’s or DNP, national certification in family, adult, or gerontological primary care, and active licenses.
Preferred experience in primary care and understanding of coding and risk assessments are advantageous.
