Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide equitable, value-based primary care for adult Medicare patients in outpatient settings.

Independently conduct office visits, wellness exams, and acute care with a focus on building patient relationships.

Coordinate care with specialists, testing facilities, and care teams, while leading population health initiatives.

Educate patients, assist with triage and outreach, and participate in community activities.

Perform home visits as needed and collaborate closely with operational leadership.

Qualifications: Master’s degree in Nursing, national certification in family, adult, or gerontological primary care, active NP license, DEA license, and US work authorization.

Preferred: Experience in primary care, knowledge of HCC coding, ICD-10, and passion for team-based, population health models. Bilingual skills are a plus.
