Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in an outpatient setting.

Provide comprehensive primary care, including routine and urgent visits, wellness assessments, and health education.

Collaborate with a multidisciplinary team, coordinate care with specialists, and participate in population health initiatives.

Conduct home visits and support patient outreach efforts.

Build trusting relationships to promote patient engagement and health outcomes.

Qualifications include: a Master’s or Doctorate in Nursing, national certification in relevant specialties, active licensure, and DEA registration. Experience in primary care and understanding of coding and documentation are preferred.
