Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide high-quality, equitable primary care to Medicare adults in an outpatient setting.

Independently see patients, conduct routine and acute visits, and perform wellness exams with holistic documentation.

Collaborate with a multidisciplinary team including physicians, nurses, and care managers to coordinate care and support population health initiatives.

Educate patients, participate in outreach, and conduct home visits as needed.

Report to the Center Medical Director and work closely with operational leadership.

Qualifications: Master’s or DNP, national certification in family, adult, or gerontological primary care, active NP license, DEA license, and US work authorization. Preferred experience in primary care and population health documentation.

Position details: Full-time, in-person, with a pay range of $105,000–$148,000 annually.
