Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to provide equitable, value-based primary care for adults on Medicare in outpatient settings.

Independently conduct routine and acute office visits, wellness exams, and health assessments.

Coordinate care with specialists, testing facilities, and agencies while building trusting patient relationships.

Lead population health initiatives and assist with patient outreach and education.

Participate in promotional activities and conduct home visits as needed.

The role reports to the Center Medical Director and involves collaboration with a multidisciplinary team.

Qualifications: Master’s or DNP in Nursing, national certification (e.g., Family, Adult-Geriatric, or Gerontological Nurse Practitioner), active licenses, and US work authorization.

Preferred: Primary care experience, knowledge of HCC/ICD-10, team-oriented attitude, bilingual skills.
