Seeking a Full-Time Nurse Practitioner (NP) for the Post-Acute Care team in Sumter, SC. This Monday–Friday role offers
consistency, clinical autonomy, and a supportive team environment.

• Schedule: Full-time, Monday–Friday, 8:00 AM–4:00 PM
• In-person care at local Post-Acute & Skilled Nursing Facilities
• Collaborative care with attending physicians and on-site teams

Benefits include a salary of $100,000–$120,000, annual bonuses, a CME stipend, paid holidays,
and comprehensive health, dental, vision, 401(k), and PTO.
Qualifications required: active South Carolina NP license, board certification, strong clinical skills,
and passion for geriatric and preventative care. Experience in post-acute or outpatient care is preferred.
