Job Description

This role is for a Wound Care Nurse Practitioner or Physician Assistant dedicated to providing specialized wound management for post-acute patients.

Conduct comprehensive patient assessments and develop individualized treatment plans.

Monitor and adjust treatments, educate patients and families on wound care, and collaborate with healthcare teams.

Maintain accurate documentation and stay updated on wound care advancements.

Participate in quality improvement initiatives.

Qualifications include a Master’s degree, current licensure in Ohio, preferred wound care certification, and experience in outpatient or primary care settings. Strong communication skills and familiarity with electronic health records are essential.

Offering full-time or part-time roles with a salary range of $115,000-$130,000, benefits include flexible scheduling, health insurance, and paid time off. The position involves on-the-road work, contributing to high-quality patient care and improved outcomes.
