Palliative Nurse Practitioner

Job Description

We are seeking a compassionate Palliative Care Nurse Practitioner to join our team, providing holistic care to patients with serious illnesses.

Conduct comprehensive assessments to address physical, emotional, and psychosocial needs.

Develop individualized care plans with interdisciplinary teams.

Manage complex symptoms and provide symptom relief, including pain management.

Educate patients and families on treatment options, advanced care planning, and end-of-life decisions.

Facilitate communication among healthcare providers, patients, and families.

Participate in research and maintain accurate documentation in compliance with regulations.

Coordinate resources and services across care settings.

Qualifications include a master’s degree in Nursing, active NP certification, and experience in palliative or hospice care. Proficiency with EMR systems and strong communication skills are essential.
