Job Description

We seek a compassionate Palliative Care Nurse Practitioner to improve quality of life for patients with serious illnesses.

  • Conduct comprehensive assessments addressing physical, emotional, and psychosocial needs.
  • Develop and implement personalized care plans with interdisciplinary teams.
  • Manage complex symptoms, including pain and acute care interventions.
  • Educate patients and families on treatment options, advance care planning, and end-of-life decisions.
  • Facilitate communication among healthcare providers, patients, and families.
  • Participate in clinical research to enhance palliative care practices.
  • Maintain accurate documentation in compliance with regulations.
  • Coordinate resources and services across care settings.

  • Qualifications include a Master’s in Nursing with a focus on Palliative Care, active Nurse Practitioner certification, and relevant experience. Proficiency with EMR systems and knowledge of pain management techniques are essential. Strong communication and interpersonal skills are required.
