Job Description

We are seeking a mission-driven Physician to provide high-quality, person-centered care in senior living communities and skilled nursing facilities.

  • Deliver compassionate medical care through history, physicals, and routine rounds.
  • Collaborate with APPs to manage complex patient cases and ensure compliance.
  • Mentor APPs to enhance team performance and care quality.
  • Drive improvements in patient outcomes by closing quality gaps.
  • Advocate for patients and represent the organization’s values in communities.
