Job Description
The Palliative Care Advanced Practice Provider (APP) is a key member of the interdisciplinary team delivering goal-concordant care in long-term and skilled nursing facilities.
Conduct comprehensive assessments and manage symptoms such as pain, dyspnea, and distressing conditions. Engage residents and families in goals of care and advance care planning, documenting discussions per guidelines. Facilitate hospice referrals and end-of-life planning, supporting transitions and family needs. Collaborate with healthcare teams, including social workers and nursing, to ensure coordinated care. Provide education to residents, families, and staff on disease progression and comfort strategies. Maintain thorough documentation, participate in quality improvement, and adhere to protocols. Maintain licensure and pursue ongoing professional development. Position is full-time, based in facilities, working four days a week with on-site duties and rotating remote call.
