Job Description
The Family Medicine Physician, supervised by the Lead Physician and Chief Medical Officer, provides comprehensive primary and urgent care to a diverse patient population, ensuring high-quality services aligned with protocols and policies.
Leads the care team using the Patient Centered Medical Home model, performs examinations, develops treatment plans, and manages acute/chronic conditions across all ages.
Ensures documentation compliance, orders/tests results interpretation, and delivers care via in-person, telehealth, or phone modalities.
Supports patient education, health promotion, and preventive screenings, while coordinating referrals and follow-up care.
Ensures regulatory compliance, participates in quality improvement, and maintains clinical records per standards.
Fosters effective communication within the team, builds relationships with external agencies, and maintains patient confidentiality.
Manages clinic resources efficiently, completes accurate documentation for billing, and adheres to fiscal practices.
