Job Description

We are seeking an experienced Physician to join our expanding post-acute care team, providing primary care in Skilled Nursing Facilities, Assisted Living, and Long-Term Care settings.

  • Deliver comprehensive care, including assessments, chronic disease management, and care coordination.
  • Work with a team of Nurse Practitioners and staff to focus on patient-centered, thoughtful medicine.
  • Enjoy clinical autonomy, flexible scheduling, minimal on-call duties, and opportunities for leadership roles.
  • Qualifications include MD/DO with Minnesota license, Board Certification in Family or Internal Medicine, and preferably 2+ years of geriatrics or primary care experience.

    • Salary: $220,000–$270,000/year.
    Location: Minneapolis, MN (required to relocate).
    Travel: Up to 25% preferred.
