Job Description

We are seeking a dedicated Physician to provide high-quality, person-centered care for seniors in community settings, mentor Advanced Practice Providers, and improve patient outcomes.

• Conduct history and physicals, perform routine rounds, and manage complex cases.
• Collaborate with APPs to ensure clinical excellence and compliance.
• Mentor and oversee APP performance to elevate care quality.
• Drive measurable improvements by closing quality gaps.
• Advocate for patients and embody the organization’s mission in the community.
