Seeking a Primary Care Physician to join a collaborative team within a patient-centered medical home model.

Our practice offers strong support from pharmacists and nurse case managers for high-risk patients, with seamless care coordination with specialists.

Features include on-site diagnostic services (x-ray, ultrasound, 3D tomosynthesis), lab draws, and outpatient procedures such as suturing and treatments. Adjacent facilities provide after-hours urgent care, ensuring smooth patient transitions.

Additional on-site providers include Ob/Gyn and dermatology, along with physical therapy, fostering comprehensive care.

Projected Compensation: From $250,000/year
