We serve vulnerable individuals with complex, chronic conditions, providing comprehensive, community-based care focused on improving health outcomes.

Our team surrounds members with medical, social, and support services—including a pharmacy, labs, and urgent care—under one roof, emphasizing “care beyond medicine.”

Responsibilities include managing patients with multiple conditions, leading multidisciplinary teams, developing transitional care plans, and managing acute exacerbations within the center. The role involves coordinating discharges, preventing readmissions, educating members, and practicing evidence-based medicine aligned with the PCMH model.

Qualifications include being a licensed, board-certified physician in Internal Medicine or Family Practice, with BLS, DEA, and experience in population health and complex patient populations preferred.
