Job Description
This full-time, in-person role focuses on managing complex, chronic medical conditions within a comprehensive care setting.
Lead a multidisciplinary team to develop and implement individualized care plans, ensuring seamless transitions across hospital, ER, and home care. Manage acute exacerbations of chronic illnesses, providing urgent interventions and preventing readmissions. Communicate with care facilities to facilitate appropriate discharges and follow-up care. Educate members on disease progression and advance directives, practicing evidence-based medicine aligned with the PCMH model. Qualifications include licensure, board certification in Internal Medicine or Family Practice, BLS, DEA, and EMR experience. Preferred experience includes population health management and working with complex patient populations.
