Job Description

This full-time, in-person role focuses on managing complex, chronic medical conditions within a comprehensive care setting.

  • Lead a multidisciplinary team to develop and implement individualized care plans, ensuring seamless transitions across hospital, ER, and home care.
  • Manage acute exacerbations of chronic illnesses, providing urgent interventions and preventing readmissions.
  • Communicate with care facilities to facilitate appropriate discharges and follow-up care.
  • Educate members on disease progression and advance directives, practicing evidence-based medicine aligned with the PCMH model.
  • Qualifications include licensure, board certification in Internal Medicine or Family Practice, BLS, DEA, and EMR experience. Preferred experience includes population health management and working with complex patient populations.
