Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner is an APRN or Physician Assistant working independently to manage patients in PALTC settings.

  • Provide comprehensive assessments, diagnose, treat illnesses, prescribe medications, and develop care plans.
  • Collaborate with facility leadership and interdisciplinary teams to ensure holistic care and effective communication.
  • Maintain compliance with regulations, document accurately, and participate in quality improvement efforts.
  • Ensure patient safety through infection control, health education, and adherence to best practices.
  • Work independently, make clinical decisions, and respond swiftly in emergencies.
  • Utilize EHR and telemedicine tools effectively.
  • Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and board certification.
