Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for independently managing patient care in PALTC settings.

  • Conducts physical exams, diagnoses, treats, and prescribes medications for acute and chronic conditions.
  • Develops care plans, provides preventive services, and monitors patient health.
  • Works collaboratively with facility leadership, interdisciplinary teams, and families to ensure holistic care.
  • Ensures compliance with regulations, maintains accurate documentation, and participates in quality improvement.
  • Participates in infection control, health education, and safety protocols.
  • Works independently with strong clinical decision-making, communication, and technology skills.
  • Requires at least 2 years of experience, relevant licensure, and board certification.
