Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for independently managing patient care in PALTC settings.
Conducts physical exams, diagnoses, treats, and prescribes medications for acute and chronic conditions. Develops care plans, provides preventive services, and monitors patient health. Works collaboratively with facility leadership, interdisciplinary teams, and families to ensure holistic care. Ensures compliance with regulations, maintains accurate documentation, and participates in quality improvement. Participates in infection control, health education, and safety protocols. Works independently with strong clinical decision-making, communication, and technology skills. Requires at least 2 years of experience, relevant licensure, and board certification.
