Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for delivering independent, comprehensive medical care in PALTC facilities.
- Manage patient health by conducting examinations, diagnosing, treating illnesses, prescribing medications, and providing preventive care.
- Work autonomously within scope, making clinical decisions and escalating to specialists when needed.
- Build collaborative relationships with facility leadership, staff, families, and external providers for holistic care.
- Ensure compliance with regulations, maintain accurate records, and participate in quality initiatives.
- Participate in infection control, health education, and safety protocols.
- Demonstrate strong communication, leadership, and technology skills, including EHR and telemedicine proficiency.
- Work in person at statewide long-term care facilities, requiring physical stamina and quick response abilities.
Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification.
