Job Description
The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, either a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.

Key duties include comprehensive assessments, individualized care planning, advanced wound treatments, collaboration with healthcare teams, prevention strategies, staff education, documentation, quality improvement, and resident advocacy. The AP also manages infections, ensures regulatory compliance, and engages families in wound care education.
Qualifications include a relevant degree (e.g., MSN, DNP, NP, PA), at least 2 years of experience (preferably in long-term care), and board certification (e.g., AANP, ANCC, PA-C). Specialty wound care certification is strongly preferred.
