The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional specializing in wound assessment, treatment, and prevention in a long-term care setting.
Responsibilities include comprehensive resident evaluations, developing and updating individualized wound care plans, performing advanced wound treatments, and collaborating with healthcare teams. They implement evidence-based prevention strategies, provide staff education, and ensure documentation compliance. The AP serves as a clinical resource, manages infections, and supports quality improvement initiatives while advocating for resident dignity and autonomy. Key skills include excellent communication, leadership, proficiency with electronic health records, and physical ability to respond quickly to resident needs. Qualifications require a relevant advanced degree, at least 2 years of experience, board certification, and preferred specialty wound care certification.
