Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a full-time clinical professional, such as a Nurse Practitioner or Physician Assistant with specialized wound management training, responsible for assessing, treating, and preventing wounds in long-term care residents.

The role aims to reduce complications, improve care quality, enhance patient and family experience, prevent avoidable hospitalizations, and lower costs.

Key duties include comprehensive assessments, individualized care planning, advanced wound treatments, collaboration with interdisciplinary teams, prevention strategy development, staff education, detailed documentation, quality improvement participation, consultation, infection management, regulatory compliance, patient advocacy, and resident/family education.

Excellent communication, leadership, proficiency with electronic health records, and physical ability to respond quickly are essential.

Qualifications require a relevant degree, at least 2 years of experience, board certification (e.g., AANP, ANCC, PA-C), and preferred specialty wound care certification. Benefits include health insurance, 401(k), paid time off, and more.
