Seeking Board Certified Geriatricians, Internists, and Family Practitioners to deliver comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Our team cares for a range of patients, from short-term rehab to complex geriatric cases, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.
As a Continuity Physician, responsibilities include providing face-to-face primary care, participating in educational sessions, and collaborating with nurse practitioners and physician assistants.
