Job Description
Seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Providers will deliver face-to-face primary care, participate in educational sessions and outcomes reviews, and collaborate with nurse practitioners and physician assistants. The role supports a structured care model aimed at enhancing clinical, cultural, and financial performance of partner facilities.
