Post Acute Care Physician / Primary Care Physician – Berlin, VT

Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

Candidates will deliver face-to-face primary care, participate in educational activities, and collaborate with healthcare teams to improve patient outcomes and facility performance.
