Post Acute Care Physician / Primary Care Physician – Berlin, VT

Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

• Deliver face-to-face primary care services to diverse patient populations, including complex geriatric cases.
• Participate in monthly educational sessions and clinical outcomes reviews.
• Collaborate with Nurse Practitioners and Physician Assistants to enhance patient care.
