Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Our team delivers care across diverse facilities, from short-term rehab to complex geriatric patients in skilled nursing homes, aiming to enhance access, remove barriers, and achieve optimal clinical outcomes.
As a member, you will offer face-to-face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
