Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Responsibilities include face-to-face primary care, participation in educational sessions, and collaboration with healthcare team members. The role aims to improve access, clinical outcomes, and facility performance through a structured care model.
