Post Acute Care Physician / Primary Care Physician – Providence, RI

Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

Deliver care to a range of patients from short-term rehab to complex geriatric cases, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.

As a team member, you’ll offer face-to-face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
