Job Description
Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Farmington, ME, and nearby areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 3-4 days weekly.
Examine patients, review medical records, order diagnostic tests, diagnose conditions, and prescribe treatments and medications.
Educate patients and families on health conditions, prevention, and wellness practices.
Maintain accurate documentation and promote patient health and safety.

Work is in person, with a focus on delivering care directly at patients’ residences and facilities.
