Job Description
Seeking a full-time or part-time Nurse Practitioner or Physician Assistant to conduct primary care visits in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.
Provide primary care, medication reconciliation, and annual wellness visits 3-5 days weekly.
Examine patients, review records, order diagnostic tests, diagnose conditions, and propose treatments.
Prescribe medications, educate patients and families, and promote health practices.
Maintain accurate documentation and oversee patient health management.
