Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to perform primary care visits in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.

  • Provide primary care, medication reconciliation, and annual wellness visits 3-5 days weekly.
  • Examine patients, review records, order and analyze diagnostic tests, diagnose conditions, and prescribe treatments and medications.
  • Educate patients and families on health conditions, promote health practices, and oversee overall patient wellness.
  • Maintain accurate records, inform patients about medications, and stay updated professionally.
