Job Description
Seeking a full-time or part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and surrounding areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 3-5 days per week.
Examine patients, review medical records, order diagnostic tests, diagnose conditions, and prescribe treatments and medications.
Educate patients and families on health conditions, prevention, and care plans.
Maintain accurate documentation and promote overall patient health.
Offer flexible scheduling, malpractice coverage, comprehensive benefits, and competitive pay up to $140,000/year.
Work in person with a supportive team, utilizing an online patient portal for updates and communication.
