Job Description
Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing Facilities and Assisted Living in Keene, NH, and nearby areas.
Perform primary care visits, medication reconciliations, and annual wellness exams 3-5 days weekly.
Examine patients, review medical records, order and interpret diagnostic tests, diagnose conditions, and prescribe treatments and medications.
Educate patients and families on health conditions, prevention, and wellness practices.
Maintain accurate records, promote good health, and oversee overall patient health.
