Job Description

We are seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Portsmouth, NH, and surrounding areas.

Responsibilities:

  • Conduct patient examinations, review medical records, and order diagnostic tests
  • Diagnose and treat health conditions and illnesses
  • Prescribe medications and manage treatment plans
  • Perform wellness visits and medication reconciliations
  • Educate patients and families on health conditions, prevention, and disease management
  • Maintain accurate medical documentation and promote overall patient health
  • Stay updated with professional knowledge and conduct research
