Seeking a Full-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Portsmouth, NH, and surrounding areas.
Responsibilities:
Conduct patient examinations, review medical records, and order diagnostic tests.
Diagnose and treat health conditions, prescribe medications, and develop care plans.
Perform annual wellness visits, medication reconciliation, and educate patients and families.
Maintain accurate records, promote health practices, and oversee patients’ overall well-being.
