The Primary Care Physician is a vital member of a multidisciplinary team providing comprehensive healthcare in an urban community health center serving a diverse population.
She/he will develop and support a team-based, patient-centered preventive and chronic care model, emphasizing continuous improvement.
Qualifications include MD/DO with residency in Internal or Family Medicine, board eligibility, active MA licensure, and bilingual Spanish preferred.
The role involves managing a patient panel, reviewing labs and imaging, coordinating referrals, ensuring accessibility, documenting care, and promoting community engagement.
Responsibilities also include quality improvement, participating in projects, and maintaining professional standards, with a focus on efficiency, communication, and patient-centeredness.
Compensation ranges from $175,000 to $230,000 plus benefits, with a commitment to diversity and equal opportunity.
